Dental implants replace lost teeth in the most natural way possible. The implant itself replicates the root of the tooth. The dental crown is attached to it with the help of a special abutment. The prices for implants are therefore part of the costs. The final cost of an implant-supported tooth is made up of the total price for the implant, the abutment, and the crown on the implant (bonded or screw-retained).
Implant-supported dentures and other more complex constructions are calculated individually — depending on the technology used and the number of implants inserted.
The prices for implants are often also influenced by the need for bone or gum augmentation. These are additional procedures to improve the anatomical conditions required for a lasting cosmetic treatment effect.

These Regulations define the rights and obligations of patients of Stomatologia na Podzamczu (hereinafter referred to as the Clinic) in relation to making appointments and providing medical services. The Regulations also set out the principles of appointment confirmation, booking fees and the consequences of failure to cancel an appointment on time. Acceptance of the regulations obliges the parties to apply them and binds them, as they form part of the contract for the provision of medical services.
(1) Medical services are provided on working days at the times individually determined according to the clinic's work organization.
(2) The clinic provides medical services for a fee in accordance with the applicable price list. Information on the prices of the services can be obtained from the clinic staff. The price list and the list of services offered are available at the registration desk and on the website www.stomatologia.szczecin.pl and www.implantologia.szczecin.pl.
(3) Paid medical services are provided without a referral, except in cases specified in separate regulations.
(4) Diagnostic X-ray examinations are performed on the basis of a referral, except for intraoral X-rays. The patient is obliged to present the referral each time before the service is provided.
(5) The services may only be provided by healthcare professionals who have the appropriate qualifications to provide the services, as confirmed by diplomas and certificates, and who meet the relevant health requirements.
(6) The provision of healthcare services is subject to prior registration at a specific time in accordance with the clinic's schedule. The registration can be made by the patient, his/her legal representative or a person authorized by the patient:— directly at the clinic

(7) When making a booking, the patient, his/her legal representative or a person authorized by the patient is obliged to provide correct data of the patient required for booking an appointment and issuing a patient card, including first name, last name, PESEL number, date of birth and residential address, at the request of the person making the booking. It is also necessary to provide data enabling direct contact with the patient in organizational matters related to the provision of services, such as the contact's telephone number or e-mail address. Detailed information on data processing is available at the clinic and at www.stomatologia.szczecin.pl.
(8) After an initial consultation, the reception staff make appointments for patients to see the doctor responsible for a particular treatment. Consultation appointments are scheduled for 30–60 minutes, dental examinations for 30 minutes, endodontic treatments for 60 to 120 minutes. The duration of a surgical visit is determined individually, depending on the extent of the procedure. The duration of subsequent visits is determined by the doctor, depending on the type of treatment planned.
(9) Doctors do not offer consultations by telephone or e-mail.

(11) The day before the appointment, a text message will be sent by 1.30 p.m. asking for confirmation of the appointment on the following day. If there is no reply by 3.30 p.m., the clinic staff will contact the patient by telephone to confirm attendance at the appointment on the following day. If the appointment cannot be confirmed by 4.00 pm, the appointment will be canceled. In this case, the clinic may send out a cancellation notice. Implantology appointments are confirmed two days in advance.
(12) The Clinic reserves the right to change or cancel an appointment for the provision of services in random situations over which the Clinic has no control. The patient shall be informed of the change or cancellation of the appointment immediately by text message and/or telephone notification by an employee of the Clinic.
(13) The patient must cancel an appointment that he/she is unable to keep at least 24 hours before the scheduled appointment. Late cancellation of an appointment means that the appointment is canceled in violation of the appointment.
(14) As a condition of accepting the booking of a consultation appointment, the Clinic is entitled to collect a deposit in connection with the appointment until full payment of the scheduled treatments/services.
(15) The fee can be paid in person, by bank transfer or via the PayU platform (via a generated link sent in an SMS or email message, or via the website www.stomatologia.szczecin.pl or www.implantologia.szczecin.pl).
(16) The deposit will be deducted from the price of the service in the event of a visit.
(17) The deposit shall be refunded if the patient cancels the appointment within the period specified in point 13. If, in the case of a timely canceled appointment, damage has been caused by the production of a custom-made product, the clinic has the right to deduct the costs for the production of the custom-made product from the deposit.
(18) The deposit will not be refunded if the patient does not show up for an appointment or if the patient cancels in breach of the appointment. In this case, the deposit represents compensation for keeping the practice ready to provide the service and for the lost benefit that arises from the fact that the patient's appointment remains unfilled, i.e., is not attended by another patient.
(19) Failure to cancel an appointment, or cancellation with violation of the appointment that was not covered by the deposit, obliges the patient to pay the clinic the flat-rate compensation fee referred to in point 18. This fee is in the amount of PLN 200 for each booked appointment. The fee shall be added to the cost of the next visit, the price of which shall be increased by the amount of the compensation.
(20) The clinic is entitled to exempt the patient from paying the compensation fee and to waive the retention of the deposit in specially justified cases concerning the patient and at the patient's request.
(21) The failure to cancel an appointment or the cancellation in violation of the deadline specified in Section 13 also entitles the Clinic to refuse to provide further medical services in the future, unless it is an emergency for life and health.
(22) Cancellation of an appointment by the clinic entitles the patient to a refund of the deposit, provided that this has been collected beforehand. If the patient does not give written instructions to this effect, the deposit will automatically be credited to the next appointment.
(23) In the case of accepted treatment plans, the payment made by the patient for the execution of the plan constitutes a down payment within the meaning of Article 394 § 1 of the Civil Code.
(24) The costs for the treatment are based on the price list valid at the time the plan is drawn up. If the price list changes after the treatment plan has been drawn up, the treatment plan will be carried out according to the changed prices. The patient acknowledges that the price list may change during the course of treatment. The healthcare provider shall inform the patient of any change to the price list that results in an increase in the cost of the treatment plan. The information is communicated directly to the patient in a conversation with the staff of the treatment facility and in the usual manner. The patient has the right to withdraw from the contract after the prices have been changed from those previously agreed in the treatment plan. The right of withdrawal expires at the latest with the first visit during the new price list and after the patient has been informed of his right of withdrawal. The validity of the treatment plan may be limited in time. The treatment plan may provide for discounts for admission/full payment.
(25) The process of providing healthcare services begins at the registration desk, where patients register their appointment. At the registration desk, the patient must(a) complete and sign a registration form or present an identity document,b) a referral for medical services, if he/she has one
(26) Due to the special nature of the medical services, the fee is paid by the patient immediately after the visit to the office on the basis of the information provided by the doctor. After payment of the fee, the patient receives a receipt/invoice.
(27) The clinic reserves the right to refuse to provide services requiring an appointment to patients who have not paid for previously provided medical services until the debt is settled, unless it is an emergency for life and health.
(28) The patient is obliged to check the accuracy of the personal data on the forms issued by the clinic staff. If he/she has any objections to the accuracy of the personal data, he/she must communicate them before using the medical service.
(29) At the registration desk, the patient must make a declaration that he/she authorizes other persons to obtain information about his/her state of health and to inspect medical documents. In addition, the patient must declare that they have read the information form on the principles of data processing.
(30) Consent to the procedure must be given by the patient each time before the procedure begins. Refusal to sign the consent form entitles the clinic to refuse the service, unless the patient is in a life-threatening or health-endangering condition.
(31) Patients treated at the clinic are expected to attend regular, unsolicited follow-up appointments every 6 months. This is to ensure that the condition of the teeth, bone, and gums can be adequately monitored and that any hygienic or therapeutic treatments can be recommended. This will ensure that the effect of the treatment carried out is sustainable and oral health is maintained.
(32) During a medical consultation, the patient has the right to receive all necessary explanations regarding the diagnosis made and the proposed treatment.
(33) The clinic staff is not obliged to provide services to the patient regardless of the patient's behavior. Reprehensible behavior of the patient in the form of active or verbal aggression or behavior that violates the provisions of the Regulations shall be punished by refusal of health care services. Gross violations of the regulations entitle the clinic to remove the patient from the patient list.
(34) The clinic shall provide services to patients who have violated the order only if there is an urgent threat to life or health. Staff who provide healthcare services in a situation that precludes refusal are subject to the protection that applies to public officials on duty. The special protection of public officials leads to increased criminal liability for the patient and to prosecution of the offense committed by the public prosecutor's office.
(35) The clinic shall maintain the medical records of persons receiving health care services and shall ensure the protection of personal data contained in these records and make them accessible in accordance with the relevant legal regulations.
(36) Medical documents or examination results (X-rays) shall be issued by the Clinic staff, the patient's legal representative or a person authorized by the patient after verification of identity.
(37) The Clinic reserves the right to change the admission schedule, scope, and price list of services.
